Dealer scanned card wrong?

last night at live blackjack, i’m almost positive the dealer scanned the wrong card and didn’t notice. nobody at the table made a fuss but it definitely messed up the next hands and the rhythm of the shoe. doesn’t it seem like stuff like this should get picked up immediately, either by someone watching or some internal check? i feel like stuff like this throws off any edge you think you’re managing at these tables, especially if you’re tracking cards or running any pattern. curious how everyone else deals with it.
